Abstract Introduction Multiple studies have shown the effects of prone (PP), supine (SP) and kangaroo (KP) positions on clinical and physiological outcomes in preterm newborns, but none compared these three types of positioning between them. Objective To investigate the influence of these positionings on heart rate, respiratory rate, peripheral oxygen saturation (SpO2) and alertness status in clinically stable preterm newborns (NBs) admitted to a neonatal intensive care unit. Methods In a randomized clinical trial, clinically stable NBs with gestational ages from 30 to 37 weeks who were breathing spontaneously were allocated in three positioning groups: PP, SP and KP. Heart rate, breathing frequency, SpO2 and alertness status were evaluated immediately before and after 30 minutes of positioning. Results In all, 66 NBs were assessed (corrected age: 35.48 ± 1.94 weeks; weight: 1840.14 ± 361.09 g), (PP: n = 22; SP: n = 23; KP: n = 21). NBs in the PP group showed a significant improvement in peripheral SpO2 (97.18 ± 2.16 vs 95.47 ± 2.93 vs 95.57 ± 2.95, p = 0.03) compared with the SP and KP groups. Conclusion In clinically stable preterm NBs, the PP was associated with better peripheral oxygen saturation than the SP or KP. In addition, there was a reduction in heart rate within prone position group and in the KP group there was an increase in the number of NBs in the deep sleep classification.

Resumo Introdução Vários estudos têm demonstrado os efeitos das posições prona (PP), supina (SP) e canguru (KP) sobre os resul-tados clínicos e fisiológicos em recém-nascidos prematuros, mas nenhum comparou esses três tipos de posicionamento. Objetivo Investigar a influência desses posicionamentos na frequência cardíaca, frequência respiratória, saturação periférica de oxigênio (SpO2) e estado de alerta em recém-nascidos pré-termo (RN) clinicamente estáveis internados em uma unidade de terapia intensiva neonatal. Métodos Em um ensaio clínico randomizado, RN clinicamente estáveis com idade gestacional de 30 a 37 semanas e respirando espontaneamente foram alocados em três grupos de posicionamento: PP, SP e KP. Frequência cardíaca e respiratória, SpO2 e estado de alerta foram avaliados imediatamente antes e após 30 minutos de posicionamento. Resultados Ao todo, foram avaliados 66 RNs (idade corrigida: 35,48 ± 1,94 semanas; peso: 1840,14 ± 361,09 g), (PP: n = 22; SP: n = 23; KP: n = 21). Os RNs do grupo PP apresentaram melhora significativa na SpO2 periférica (97,18 ± 2,16 vs 95,47 ± 2,93 vs 95,57 ± 2,95, p = 0,03) em comparação aos grupos SP e KP. Conclusão Em RN prematuros clinicamente estáveis, o PP foi associado à melhor saturação periférica de oxigênio do que o SP ou KP. Além disso, houve redução da frequência cardíaca no grupo de posição prona e no grupo KP houve aumento do número de RNs na classificação sono profundo.

Abstract Introduction The intervention of respiratory physio-therapy in neonatal units is in continuous development, having its own care characteristics related to the weight and gestational age of the newborn, respecting the immaturity of the organs and systems and the diseases of this patient. Through techniques, the objective is to optimize the respiratory function, assisting in the clearance of secretions, and the restoration of lung volumes. Objective To verify if the respiratory physiotherapy technique of selective insufflation alters the cerebral blood flow in premature infants under 34 weeks of gestational age. Methods This is an uncontrolled clinical trial, conducted in a Neonatal Intensive Care Unit of a level III hospital, between January 2019 and March 2020, with participation of premature newborns under 34 weeks of gestational age. All were submitted to transfontanellar Doppler ultrasonography to assess cerebral blood flow measurements, mainly the resistance index, before and after the application of the selective insufflation respiratory physiotherapy technique. Results Sixty-two newborns were included, with a mean gestational age of 29.3 ± 2.2 weeks and birth weight of 1,259 ± 388 grams. The resistance index did not change significantly (RI before: 0.55 ± 0.07; after: 0.54 ± 0.07; p = 0.06) before and after the intervention and no studied variables such as, gender, gestational age, weight, Apgar score or SNAPPE II score had an influence on cerebral blood flow measurements. Conclusion The selective insufflation technique did not alter cerebral blood flow in premature newborn infants under 34 weeks gestational age.

Resumo Introdução A intervenção da fisioterapia respiratória nas unidades neonatais está em contínuo desenvolvimento, tendo características próprias de atendimento relacio-nadas ao peso e à idade gestacional do recém-nascido, respeitando a imaturidade dos órgãos e sistemas e as doenças desse paciente. Objetivo Verificar se a técnica de fisioterapia respiratória de insuflação seletiva altera o fluxo sanguíneo cerebral de prematuros menores de 34 semanas de idade gestacional. Métodos Trata-se de um ensaio clínico não controlado, realizado em uma Unidade de Terapia Intensiva Neonatal de um hospital nível III, entre janeiro de 2019 e março de 2020, com a participação de recém-nascidos prematuros menores de 34 semanas de idade gestacional. Todos foram submetidos ao exame de ultrassonografia transfontanela com Doppler para avaliar as medidas de fluxo sanguíneo cerebral, principalmente o índice de resistência, antes e depois da aplicação da técnica de fisioterapia respiratória de insuflação seletiva. Resultados Sessenta e dois recém-nascidos foram incluídos, com média de idade gestacional de 29,3 ± 2,2 semanas e peso de nascimento de 1259 ± 388 gramas. O índice de resistência não se modificou de forma significativa antes e depois da intervenção (IR antes: 0,55 ± 0,07; depois: 0,54 ± 0,07; p = 0,06) e nenhuma variável estudada, como sexo, idade gestacional, peso, escore de Apgar ou escore SNAPPE II, teve influência nas medidas de fluxo sanguíneo cerebral. Conclusão A técnica de insuflação seletiva não alterou o fluxo sanguíneo cerebral de recém-nascidos prematuros menores de 34 semanas de idade gestacional.

RESUMO Objetivo: Investigar a viabilidade do índice bispectral na avaliação do sono de pacientes criticamente enfermos lúcidos e a associação entre os parâmetros do sono aferidos por esse índice, pelo Questionário de Sono de Richards-Campbell e pelo ruído ambiental. Métodos: Estudo observacional transversal que avaliou indivíduos adultos criticamente enfermos com doenças de gravidades baixa ou moderada. Foram aferidos: volume e tempo total de sono, volume e tempo de sono profundo, volume e tempo de sono contínuo, latência para o início do sono e pressão sonora ambiental. A percepção subjetiva do sono foi registrada com o Questionário de Sono de Richards-Campbell nas manhãs subsequentes às noites de observação. Resultados: Os pacientes apresentaram tempo total de sono reduzido (234 minutos), predominância dos estágios superficiais e pouco sono profundo (1,7 minutos). Os volumes de sono total, profundo e contínuo foram 3.679, 9,4 e 3.143 índice bispectral versus minutos, respectivamente. A latência para o sono foi de 94 minutos. O escore médio do Questionário de Sono de Richards-Campbell foi 57,9. Foram observadas correlações de magnitudes fracas entre volume total, tempo total, volume contínuo e os domínios do Questionário de Sono de Richards-Campbell profundidade do sono, qualidade geral de sono e escore total; e correlações de magnitudes moderadas entre volume total, tempo total, volume contínuo e o domínio ocorrência de despertares. Conclusão: O índice bispectral é um instrumento com viabilidade limitada para monitorar o sono de pacientes lúcidos e com enfermidades de gravidades baixa e moderada na unidade de terapia intensiva, e pacientes que apresentaram maior volume, tempo total e volume de sono contínuo tiveram melhor percepção global do sono.

Abstract Objective: To investigate the viability of the bispectral index in the sleep evaluation of critically ill patients and to quantify the associations of sleep parameters measured by this index with the Richards-Campbell Sleep Questionnaire and environmental noise. Methods: This was a cross-sectional observational study that evaluated critically ill adults with diseases of low or moderate severity. The following were measured: total sleep volume and time, deep sleep volume and time, continuous sleep volume and time, sleep onset latency, and environmental sound pressure level. The subjective perception of sleep was evaluated with the Richards-Campbell Sleep Questionnaire the morning after each night of observation. Results: Patients had a low total sleep time (234 minutes), a predominance of superficial sleep stages, and little deep sleep (1.7 minutes). The total, deep, and continuous sleep volumes were 3,679, 9.4, and 3,143 (bispectral index units × minutes), respectively. The sleep latency was 94 minutes. The mean score of the Richards-Campbell Sleep Questionnaire was 57.9. Total sleep volume, total sleep time, and continuous sleep volume were weakly correlated with the Richards-Campbell Sleep Questionnaire depth of sleep domain score, overall sleep quality domain score, and total score. Total volume, total time, and continuous volume were moderately correlated with the occurrence of awakenings domain score. Conclusion: The bispectral index is an instrument with limited viability to monitor the sleep of lucid patients and patients with low to moderate disease severity in the intensive care unit. Patients with higher total sleep volume, total sleep time, and continuous sleep volume had better overall sleep perception.

Resumo Ao nascer, o recém-nascido pré-termo (RNPT) frequentemente necessita internamento em UTI-Neonatal que, apesar de ser um ambiente de cuidados, também causa estresses ao RNPT como a dor, alterações de sono e vigília e de parâmetros fisiológicos. Objetivou-se analisar os efeitos da fisioterapia aquática sobre a dor, o estado de sono e vigília e variáveis fisiológicas de RNPT internados em UTI Neonatal. Avaliações sobre dor, estado de sono e vigília e das variáveis fisiológicas foram realizadas em três momentos: 5 minutos antes da intervenção, imediatamente após e 10 minutos após. Na intervenção, os participantes foram envoltos em tecidos macios e imersos, a nível do ombro, em água aquecida (36°C a 37,5°C). Foram realizados movimentos látero-laterais, ântero-posteriores e rotacionais. Participaram 22 RNPT. Os resultados obtidos em relação à dor e ao estado de sono e vigília mostram melhoras significativas nas reavaliações após a intervenção. As variáveis fisiológicas também sofreram mudanças significativas e mantiveram-se dentro dos parâmetros de normalidade. Esta pesquisa aponta que a fisioterapia aquática é efetiva na redução da dor, melhora do estado de sono e vigília e das variáveis fisiológicas de RNPT internados em UTI-Neonatal.

Abstract At birth, preterm newborns (PTNB) often require Neonatal-ICU hospitalization, which, despite being a care setting, also causes stress such as pain, sleep disturbances and wakefulness, and alterations of physiological parameters. The aim of this study was to analyze the effects of aquatic physiotherapy on pain, sleep disturbances and wakefulness, and physiological variables of PTNB in Neonatal-ICU. Pain, sleep disturbances and wakefulness, and alterations of physiological parameters were evaluated at three moments: 5 minutes before intervention, immediately after and 10 minutes after intervention. At intervention, participants were wrapped in soft fabric and immersed at shoulder level in warm water (36 ° C to 37.5 ° C). Sideways, forward, backward and rotational movements were performed. Twenty-two newborns participated in the study. The results obtained in relation to pain, sleep state and wakefulness showed significant improvements in reevaluations after intervention. Physiological variables also underwent significant changes and remained within normality parameters. The results indicate that aquatic physiotherapy is effective in reducing pain, improving sleep and wakefulness and physiological variables of PTNB in Neonatal-ICU.

Resumo Introdução: O envelhecimento impõe modificações na composição corporal dos indivíduos. Uma das características mais marcantes desse período é a perda progressiva de massa, força e função musculares, conhecida como sarcopenia. Objetivos: Verificar a presença de sarcopenia em idosas hígidas da comunidade utilizando combinações de diferentes métodos para avaliar massa muscular (MM) e desempenho físico, bem como se há concordância entre as combinações para diagnóstico da sarcopenia. Método: Estudo transversal com 37 idosas da Universidade Aberta da Maturidade. Os parâmetros para avaliar a MM foram: circunferência da panturrilha (CP) e massa muscular esquelética (MME) por meio da impedância bioelétrica (BIA) e índice de músculo esquelético (IME). Quanto à performance física, realizou-se a força de preensão palmar (FPP) com dinamômetro manual, velocidade de marcha (VM) em 10 metros e o timed up and go (TUG) para mobilidade funcional. Para comparar os métodos diagnósticos de sarcopenia foram empregadas quatro combinações distintas: C1= IME+FPP+VM; C2= IME+FPP+TUG; C3= CP+FPP+VM e C4= CP+FPP+TUG, de acordo com o proposto pelo consenso europeu. Para verificação da diferença entre os métodos foram aplicados os testes de Kruskal-Wallis e as comparações múltiplas para dados não pareados; e o coeficiente Kappa ponderado para avaliação do grau de concordância. Resultados: As combinações C1 e C2 identificaram pré-sarcopenia, sarcopenia e sarcopenia grave. Entre C1 e C2, evidenciou-se alta concordância (0,92), já C3 e C4 não identificaram presença de sarcopenia nas idosas. Conclusão: A CP, para avaliar a MM, pode não detectar a sarcopenia em idosas com excesso de peso. Porém, a alta concordância entre as duas combinações que utilizaram o IME e VM ou TUG, indicam a aplicação de ambas na prática clínica.

Abstract Introduction: Aging imposes changes in the body composition of individuals. One of the most striking changes during this period is the progressive loss of mass, strength and muscle function, known as sarcopenia. Objectives: To evaluate the presence of sarcopenia in healthy elderly women living in the community through a combination of different methods to assess muscle mass (MM) and physical performance, as well as analyzing whether there is correlation between the different methods of defining sarcopenia. Methods: A cross-sectional study was performed of 37 older adults of the Universidade Aberta da Maturidade (Open University for Mature Persons). The parameters used to assess MM were calf circumference (CC) and skeletal muscle mass (SMM) through bioelectrical impedance (BIA) and the skeletal muscle index (SMI) Handgrip strength (HS) using a manual dynamometer, gait speed (GS) over 10m and the timed up and go (TUG) test for functional mobility were used to measure physical performance. To compare the sarcopenia diagnostic methods four different combinations were employed: C1= SMI+HS+GS; C2= SMI+HS+TUG; C3= CC+HS+GS and C4= CC+HS+TUG, according to European consensus proposals. To verify the differences between the methods the Kruskal-Wallis test and Multiple Comparisons for Unpaired Data were applied, and the weighted Kappa coefficient was used to assess the degree of agreement. Results: The C1 and C2 combinations identified pre-sarcopenia, sarcopenia and severe sarcopenia. A close correlation between C1 and C2 was found (0.92), while C3 and C4 did not identify the presence of sarcopenia in the elderly women. Conclusion: CC used to evaluate MM was not precise in the detection of sarcopenia in overweight elderly women. However, there was a strong agreement between the two combinations using SMI and GS or TUG, suggesting that both can be effectively applied in clinical practice.
